Item Análise comparativa de perfil epidemiológico e imuno-histoquímico em pacientes jovens com câncer de mama em clínica privada(Centro Universitário do Estado do Pará, 2019-05-29) Neiva, Christianne Gomes Barros; Miranda, Ariney Costa de; Carvalho, Ana Emília Vita; http://lattes.cnpq.br/1981562999898097; http://lattes.cnpq.br/0967752058385887; Caldato, Milena Coelho Fernandes; http://lattes.cnpq.br/9477878606835309; Barreto, Bruno Acatauassú Paes; http://lattes.cnpq.br/4741205865293040; Bacelar, Herick Pampolha Huet de; http://lattes.cnpq.br/3310315262508382Breast câncer in Young people is considered an uncommon disease accounting for less than 7% for all women diagnosed with câncer in developed countries. In the last decade, there have been fundamental changes in the understanding of the biology of breast cancer, which has come to be understood as a group of diseases with different molecular characteristics identified by gene expression profiles. This was a longitudinal retrospective case-control study in which 60 patients with a diagnosis of breast cancer, 20 young patients (≤ 40 years), Case group (Gca) and 40 patients (contro lgroup - CG) were evaluated, These groups were subdivided into two age groups: Groups 1 (GC1) with patients aged 41-59 years and Control groups 2 (GC2) with patients aged 60 or over. The patients underwent oncologic surgery, in the period between May/2013 and October/2018. The mean age of the participants was 34.2 years; 85% had symptomatic diseases; the mean tumor size was 2.6cm; 20% had a first-degree family history, and 35% had loco regional lymph nodes in the study. Histopathological examination. Regarding Postoperative Staging, 1A was more frequent. The luminal profile A was found in 38.8% of the patients, followed by luminal B in 33.3% and triple in 27.8% in the case group. Differing from the literature, in which predominance of the B-luminal profile occurs. We conclude that the sample studied presented fewer aggressive cases than found in the literature in this age group. The diagnosis of breast câncer is a challenge in this age group because women do not usually do mammographic screening before 40 years. With the purpose of drawing attention and orientation regarding breast cancer in Young women and guiding health professionals, a primer was produced with the purpose of encouraging, whenever possible, the early diagnosis and better therapeutic conduct of the patients.Item Competências clínicas essenciais em endocrinologia e metabologia para o médico generalista(Centro Universitário do Estado do Pará, 2021) Bastos, Fabíola de Arruda; Caldato, Milena Coelho Fernandes; http://lattes.cnpq.br/9477878606835309; Barreto, Bruno Acatauassú Paes; http://lattes.cnpq.br/4741205865293040; Libonati, Rosana Maria Feio; http://lattes.cnpq.br/3818175484709618; Conde, Simone Regina Souza da Silva; http://lattes.cnpq.br/3375802140515152; Del Nero, Luciana Brandão Carreira; http://lattes.cnpq.br/5056614008350803Introduction: Teaching by Competencies mobilizes Knowledge, Skills and Attitudes. In the medical field, it is important to define the competences necessary for a general practitioner, contributing to the training of professionals more prepared for the job. A competence matrix expresses the collective consensus about the essential content for the student, being an instrument able to guide the teaching-learning processes. Objective: To develop a proposal for a Matrix of Essential Clinical Competencies in the Area of Endocrinology and Metabolism for the General Practitioner, to be achieved at the end of the undergraduate course in Medicine. Methodology: For the accomplishment of this work, the Delphi methodology was applied, in order to obtain consensus about the Essential Clinical Competencies. The study was divided into four phases: bibliographic survey and construction of the Pilot Matrix; opinion consultation with 07 local judges; analysis of the results obtained after the 1st evaluation cycle and elaboration of a questionnaire for a 2nd evaluation cycle; application of the questionnaire, analysis of responses and preparation of the Final Competency Matrix. Results: The Pilot Matrix was composed of 23 competencies, defined based on the main sub-areas of Endocrinology and Metabology, covering Knowledge, Skills and Attitudes. After the 1st evaluation cycle, alterations were suggested in 14 competences, totaling 22 alteration proposals. After the 2nd evaluation cycle, there was consensus on 16 of the 22 suggested changes, 05 for Increasing the Complexity of Competence, 07 for Reducing the Complexity of Competence and 04 for Changing the Competence. There was no situation of Exclusion from Competence. Conclusion: The elaboration of a proposal for a Matrix of Essential Clinical Competencies in the Area of Endocrinology and Metabology for the General Practitioner, to be achieved at the end of the graduation in Medicine, is fundamental, due to the increasing frequency of endocrine and metabolic diseases in the world population, the shortage of specialists and the reduced amount of adequate material on the subject. In this way, the Matrix proposal contributes to contemporary medical education based on Competencies, as well as to the qualified teaching of Endocrinology in undergraduate Medicine.Item Competências na coleta do exame preventivo do câncer do colo uterino de graduandos do curso de enfermagem(Centro Universitário do Estado do Pará, 2020) Olivi, Clara Ozeny Lima; Caldato, Milena Coelho Fernandes; http://lattes.cnpq.br/9477878606835309; Pereira, Cynthia Mara Brito Lins; http://lattes.cnpq.br/3186657130674078; Dantas, Elza Ezilda Valente; http://lattes.cnpq.br/3951303999926297; Santana, Mary Elizabeth de; http://lattes.cnpq.br/6616236152960399As a result of the screening of the precursor lesions of uterine cancer being the responsibility of the nurse in Primary Care, it is essential to deepen and appropriate the correct examination technique that detects the injury early. Therefore, when nursing students graduate, they should be able to perform the oncotic colpocytology exam. The oncotic colpocytology exam is useful in screening for cervical cancer lesions. Exam collection requires manual skill through practice and mastery of the correct technique. Therefore, the importance of the nursing student having a supervised internship in Primary Health Care to have the opportunity to acquire learning and minimal skills to perform the exam. The general objective of the present study was to identify the level of competence in the collection of the preventive exam of cervical cancer among Nursing students taking the supervised internship. Method: The study presented a quantitative approach and performed in the year 2019. 80 (eighty) students from the 9th semester of the Nursing Course at the University of the Amazon participated during three months of supervised internship in Primary Health Care. Twenty-one competencies in the collection of oncotic colpocytology exam. From the application of a checklist during the supervised internship, each student was evaluated five times. Results: The evolution of the minimum competencies of the nursing student during the five collections of the oncotic cytology exam was notorious. From the third collection, it was observed that 41.3% of students with unsatisfactory performance, there was a reduction to 5% in the fourth collection. In the fifth collection of the exam, 92.5% of students reach a satisfactory exam technique. Therefore, it suggested implementing the checklist during the collection of the oncotic cytology exam in the supervised internship, so that each student has the opportunity to make at least five collections.Item Conhecimento de profissionais de saúde na mobilização de paciente politraumatizado no âmbito hospitalar(Centro Universitário do Estado do Pará, 2022) Maciel, Flávia Lobato; Caldato, Milena Coelho Fernandes; Costa, Leonardo Ramos Nicolau da; http://lattes.cnpq.br/0412027987799407; http://lattes.cnpq.br/9477878606835309; Miranda, Ariney Costa de; http://lattes.cnpq.br/0967752058385887; Silva, Dayse Danielle de Oliveira; http://lattes.cnpq.br/1634835938202657; Ferreira, Ilma Pastana; http://lattes.cnpq.br/1650337093024641; Barreto, Bruno Acatauassú Paes; http://lattes.cnpq.br/4741205865293040; Souza, Givago da Silva; http://lattes.cnpq.br/5705421011644718This is a cross-sectional study of a quantitative nature. Real-simulation-based learning allows the trainee to be prepared on how to deal with critical situations before contacting actual patients, particularly during the emergency context. Some non-technical or behavioral skills can be especially hard to be taught and trained. For this reason, the use of specific tools like behavioral checklists or scripts, become relevant to favor this task. It is crucial to mention that such tools should also be linked to the student's reality. However, as such tools are currently unavailable in our teaching institution, we have chosen to build one, based on existing scales in the literature. Objective: The aim of this study was to create a behavioral checklist applied to the realistic simulation environment of the urgency and emergency module from CESUPA medical course. Methods: For this reason, a literature review was carried out on the undergraduate competences and currently validated scales in the health area that addressed the main behavioral skills of the medical professional. From then on, the checklist developed was evaluated by four judges and applied to the students of the boarding school in Emergency and Emergence of CESUPA. The analyzed data was treated using descriptive statistics, expressed as median, interquartile range, percentiles and 95% Confidence Interval. Results: The generated checklists have been filled by a total of 30 students, from which 28 have been selected. Attitudes related to 'communication' skills have been significantly more frequent than those related to 'teamwork'; when students' performance involved 'airways' (p = 0.0195),' polytrauma '(p = 0.0011) and 'acute coronary syndrome' (p = 0.0490). In simulated cases of 'thoracic drainage' the frequency of ‘leadership’ behavior have shown to be significantly higher than those of ‘teamwork’ (p = 0.0022). Conclusion: 1. Communication, leadership and teamwork were considered three essential behavioral skills to be applied in our tool (checklist), in order to aid realistic high fidelity simulation activities at the urgent and emergency internship of CESUPA’s medical school. 2. The checklist built as a product of this work will be used as an auxiliary tool in our scenarios for analysis and behavioral discussion during debriefing and it may also be modified, adapted and improved for posterior use in different activities in our institution. 3. We believe those studied behavioral skills might have their anticipated training in related activities through in diverse contexts in order to provide greater mastery from our students.Item Elaboração e implementação de um guia didático para construção compartilhada de projeto terapêutico singular (PTS) no ensino médico(Centro Universitário do Estado do Pará, 2024) Gaia, Gabriel Nogueira; Caldato, Milena Coelho Fernandes; http://lattes.cnpq.br/9477878606835309; Carvalho, Ana Emília Vita; http://lattes.cnpq.br/1981562999898097; Brito, Caio Vinicius Botelho; http://lattes.cnpq.br/9231585273571040; Del Nero, Luciana Brandão Carreira; http://lattes.cnpq.br/5056614008350803The Singular Therapeutic Project is an instrument used in the Family Health Strategy in Primary Health Care in Brazil, as well as in other countries, in order to understand the family composition, its interactions between family members and with members that make up its social nucleus and expectations regarding the illness process. The objectiveof this study was to propose and apply a matrix tool to ensure dialogue between the different levels of the health care network, the interdisciplinary team and, above all, guarantee the user's autonomy and protagonism in their health-disease process. The study resulted in the development of a guide titled “EDUCATIONAL GUIDE FOR SHARED CONSTRUCTION IN NETWORK CARE MANAGEMENT”, focusing on the role of the user and the attributions of each actor. In conclusion, the primary use of the Singular Therapeutic Project served to achieve the proposed objectives - to coordinate and matrix care and ensure the user's role in their health-disease process, noting that the use of this method is positive for clinical practice through the Person-centered clinical approach.Item Medicina do adolescente - matriz de competências para a graduação médica(Centro Universitário do Estado do Pará, 2024) Pinho, José Francisco Carvalho de; Caldato, Milena Coelho Fernandes; http://lattes.cnpq.br/9477878606835309; Barreto, Bruno Acatauassú Paes; http://lattes.cnpq.br/4741205865293040; Chermont, Aurymery Gomes; http://lattes.cnpq.br/4212769913736000; Franco, Mariane Cordeiro Alves; http://lattes.cnpq.br/6022804097411305; Carvalho, Ana Emília Vita; http://lattes.cnpq.br/1981562999898097; Palacios, Vera Regina da Cunha MenezesThe competency-based education aims at the formation of a professional that, beyond dominating knowledge, possesses the necessary abilities and skills to face complex challenges that will appear in professional life. The present work has as an objective to contribute for the creation of a competency matrix ordered to the Adolescent Medicine during medical graduation with the scope that the egressed may, at the end of the course, carry it out with autonomy and safety. The work in sight consists of a quantitative exploratory study, which utilized the Delphi Methodology, and was realized in four phases: first a bibliografic survey and the creation of a pilot matrix were made; in sequence a research was performed with specialists (pediatricians and adolescent medics) – first evaluation cycle of pilot matrix; in the third phase, research was done alongside a panel of local judges – second evaluation cycle of pilot matrix; and at last, the results were analysed and the definitive matrix was elaborated. At the end of the two evaluation cycles a matrix proposal composed of 24 essential competencies in Adolescent Medicine was structured upon 3 thematic axies: the first axis, containing competencies linked to adolescent care; the second, containing competencies over the main themes in Adolescent Medicine; and the third axis, with the general competencies concentrated in the acknowledgement of the main pathologies that affect adolescents. The results allowed to trace a profile of competencies in accordance with the proposed objective and will permit the fullfilment of an existent gap in professional formation, guaranting a training in Adolescent Medicine, suplying the necessity of dealing with questions more directed to psicossocial and sexual problems, and the physical and emotional developement of that part of the population which necessitates higher atention.Item O ensino de endocrinologia e metabologia pela aprendizagem baseada em problemas: a experiência de 20 turmas de medicina(Centro Universitário do Estado do Pará, 2021-04-14) Rocha, Vanessa Campos Couto da; Nunes, Leonardo Mendes Acatauassú; Caldato, Milena Coelho Fernandes; http://lattes.cnpq.br/9477878606835309; http://lattes.cnpq.br/3512764081076675; Barreto, Bruno Acatauassú Paes; http://lattes.cnpq.br/4741205865293040; Magno, Ismaelino Mauro Nunes; http://lattes.cnpq.br/3841081216551678; Liberati, Rosana Maria Feio; http://lattes.cnpq.br/3818175484709618Introduction: Problem-Based Learning (PBL) has been the preferred teachinglearning methodology among medical schools in Brazil in recent years. Thereby, there is an increasing number of Medicine courses getting experience in this method, such as the Centro Universitário do Estado do Pará (CESUPA), which the twentieth class of Medicine concluded teaching by PBL in 2020. In this context, it is relevant the analysis of how this teaching has been carried out and how it has evolved over the years, especially in areas that are interrelated with various organic systems and that involve very prevalent pathologies and that will be conducted by Primary Care Physicians, as the case of Endocrinology and Metabolism (E/M). Objectives: To describe how cognitive aspects related to E/M teaching were worked by the PBL methodology from the first to the twentieth class of CESUPA Medicine course, identifying the moments of the course in which they were studied, pointing out the changes that occurred over time and analyzing if there was a distribution of the contents so that the themes were reviewed with increasing levels of complexity. In addition, it aimed to develop, at the end, a teaching guide proposal of E/M related themes for medical courses that will use active methodologies. Methodology: A review of 360 manuals used by tutors from the first to the eighth semester of the course, in its first 20 classes, was carried out. In these manuals, there were problems to be developed, with their respective learning objectives, in addition to the objectives of the morphofunctional laboratory. Then, a critical and descriptive analysis of the E/M content present in these manuals was carried out, evaluating how the evolution of this teaching occurred over the years. Finally, the proposal for the aforementioned guide was constructed. Results: It was observed that the themes related to the Endocrinological System are distributed throughout each of the first eight semesters of the course. The initial focus is on anatomy, histology and physiology, however, from the very beginning there are citations of the pathologies that affect this system, which allows contextualization and meaningful learning. Subsequently, the subjects are revisited with progressive deepening in relation to the management of patients. Over the years, there have been significant changes, especially in the detailing and standardization of the manuals, beyond description of well-defined objectives to be achieved in the morphofunctional laboratory. Discussion: The most cited endocrine theme among the manuals is Diabetes Mellitus, which is explained for being a very prevalent pathology with high morbidity. On the other hand, obesity, which has been steadily increasing in prevalence, has also shown a progressive increase in citations in the manuals over the years. Rare pathologies were in fact not prioritized as topics for discussion, however it is suggested to expand the presence among the problems of some frequent pathologies such as thyroid nodules. Conclusion: The teaching of E/M was carried out in a well distributed way over the first four years of the CESUPA Medicine course and the way of doing it evolved significantly during its first 20 classes.Item Prevalência de sobrepeso e obesidade e sua correlação com nível de atividade física e perfil nutricional em escolares de uma instituição de ensino público municipal no estado do Pará(Centro Universitário do Estado do Pará, 2022-10-19) Batista, Elysson Raul Ferreira; Miranda, Ariney Costa de; Paes, Bruno Acatauassú; Corrêa, Cláudio Eduardo; http://lattes.cnpq.br/7448998858430931; http://lattes.cnpq.br/0967752058385887; Barreto, Bruno Acatauassú Paes; http://lattes.cnpq.br/4741205865293040; Teixeira, Cláudio Eduardo Corrêa; http://lattes.cnpq.br/7448998858430931; Cardoso, Thais Cristina Franco; http://lattes.cnpq.br/3752413982659840; Caldato, Milena Coelho Fernandes; http://lattes.cnpq.br/9477878606835309; Oliveira, Edilea Monteiro de; http://lattes.cnpq.br/2758712667035434INTRODUCTION: Several factors are involved in the etiology of overweight and obesity, among which we can mention as primary the energy imbalance between calories ingested and spent, obtained mainly from the change in eating habits in which there was a higher consumption of industrialized foods, rich in fats, sugars and salt; and consequent reduction in physical activities, bringing us more and more to sedentary lifestyle (WHO, 2014). OBJECTIVE: To verify the prevalence of overweight and obesity in elementary school students, final years (6th to 9th grade), aged between 10 and 15 years of the educational institution Laércio Wilson Barbalho Municipal School in Ananindeua - PA and its correlation with habitual physical activity and nutritional profile. METHODOLOGY: This is a descriptive cross-sectional study, conducted through an interview using a structured questionnaire for the evaluation of the following variables: schooling, gender, age, eating and leisure habits, collected through a 24-hour recall and physical activity questionnaires in the Physical Activity Questionnaire for Older Children (PAQ-C) and Physical Activity Questionnaire for Adolescents (PAQ-A). Weight and height were collected to calculate the body mass index (BMI) using a scale and a statometer. RESULTS: The study participants 266 students, with a mean age of 13.61 (±1.32), most females (51.9%), attended the ninth year (32.7%), had the ideal weight (46.2%) and was sufficiently active (52.3%), 45.2% of schoolchildren were overweight, 48.12% were insufficiently active, no relationship was observed between the above-normal body mass index (overweight and obesity) and the dietary profile in the population, no relationship was observed between the above-normal body mass index (overweight and obesity) and the level of physical activity in the population studied (Contingency coefficient=0.086, p=0.34), but a weak relationship was observed between the body mass index within normality (ideal weight) and the level of physical activity (Contingency coefficient=0.277, p<0.01). CONCLUSION: Approximately half of the participants were above the ideal weight. No relationship was found between overweight and food intake of students, but a relationship was evidenced between those with the ideal weight and the level of physical activity. A technical report was prepared with suggestions for changes in the physical education curriculum component, aiming to monitor and intervene in the level of physical activity and weight of students and proposing changes in life habits in order to minimize the possible future damage caused by overweight and inactivity.Item Reações de hipersensibilidade aos meios de contraste: avaliação do conhecimento de médicos do estado do Pará(Centro Universitário do Estado do Pará, 2021-02-19) Alcântara, Carolina Tavares de; Barreto, Bruno Acatauassú Paes; http://lattes.cnpq.br/4741205865293040; Tuji, Fabrício Mesquita; http://lattes.cnpq.br/8122254353290848; Nunes, Leonardo Mendes de Acatauassú; http://lattes.cnpq.br/3512764081076675; Caldato, Milena Coelho Fernandes; http://lattes.cnpq.br/9477878606835309Contrast media have been widely used since the 1950s to facilitate diagnostic imaging. Doctors in almost all specialties prescribe, administer or care for patients who have received some contrast medium. These differ in their chemical structure, applicability and adverse effects. Therefore, physicians should be minimally familiar with the pharmacology, administration, risks and adverse reactions associated with these agents. However, despite the emergence of evidence-based medicine, gaps in knowledge are still filled by tradition, experiences and “common sense”, giving rise to misunderstandings with the spread of myths. In this sense, this dissertation intends to evaluate the knowledge of physicians working in the state of Pará regarding adverse reactions to contrast media. Thus, an observational, analytical and cross-sectional study involving 48 physicians from different specialties, using a virtual questionnaire addressing prophylaxis, risk factors and conduct related to the development of adverse reactions to contrast media. The data were organized using Microsoft Excel® and afterwards, Cronbach's alpha and descriptive statistical analysis were performed, with a consistent answer of 0.700. The graphic analysis was performed in R and RStudio. The results indicated that all physicians agree that it is essential to question the patient's allergic history prior to the use of contrast, and 90% of them agree to question about iodine allergy. Before releasing any contrasted test, 75% request an allergist assessment when there is a history of allergy to seafood / iodine, and 86% agree that patients allergic to iodine are at increased risk for reactions to contrasts. And most doctors (86%) indicate pre-medication in those with a history of previous reaction to contrasts. It was concluded that the notion of allergies to seafood and, especially to iodine as significant risk factors for adverse reactions to contrast media, remains widespread. The act of inquiring about these allergic conditions before requesting imaging tests, as well as recurring indication of premedication may induce the idea that these situations are a potential risk to allergic reactions to contrast media, thus propagating the myth. Medical education is vital to stop the persistence of these mistakes.Item Tecnologia educacional sobre medicina do estilo de vida para profissionais de atenção primária à saúde de comunidades ribeirinhas(Centro Universitário do Estado do Pará, 2023-12-11) Lucena, Lílian Grace Moura de; Matos, Haroldo José de; Lobato, Edienny Augusta Viana Santos; http://lattes.cnpq.br/8177968072238226; http://lattes.cnpq.br/9830983900880582; Domingues, Robson José Souza; http://lattes.cnpq.br/9892890507517226; Taquette, Stella Regina; http://lattes.cnpq.br/0311106771021992; Caldato, Milena Coelho Fernandes; http://lattes.cnpq.br/9477878606835309This study focused on the development of an educational technology aimed at promoting the principles of Lifestyle Medicine (LM) in Primary Health Care of a Basic Health Unit (BHU) in the region of Ilha das Onças, Barcarena, Pará. The study highlighted the importance of LM as an evidence-based clinical discipline that promotes a healthy lifestyle. Considering the efficiency in providing public services, especially in the health area, as a constitutional principle, the proximity of the BHU services is highlighted as the only access of the riverside population to public health services. The research focused on the Ribeirinha Vital Costa Gouvêa BHU, located in an Amazonian area classified as high social vulnerability. The objectives included the development of an educational technology, analysis of the integration of LM principles in professional practice, identification of challenges and opportunities, and creation of instructional material to empower health professionals. The study was exploratory cross-sectional, using Focus Group sessions with four groups of BHU professionals and Content Analysis was applied through the IRaMuTeQ software, following the Reinert Method in the analysis of collected data. As a result, instructional materials and a teaching plan were developed for a workshop, addressing essential topics such as food pattern, physical activity, sleep, stress control, use of toxic substances, social connection, and motivational interview. These were developed based on the observations and theoretical-conceptual gaps identified by professionals during interactions with patients and discussions of focus groups, aiming to fill these gaps through permanent health education. The conclusion highlighted the need for educational strategies that incorporated both the technical approach and the emphasis on human interaction, promoting a holistic view in fostering healthy habits. The importance of effective communication, supported by Continuing Education and educational technology, was emphasized to optimize patient follow-up at the BHU.
